These 2 forms must be filled out PRIOR to your INITIAL consultation.
Please follow these instructions very carefully or else you might end up having to do it twice.

  • Download both forms to your computer and save to your desktop (working on a desktop computer is likely the best way to do this).
  • Read the disclaimer and follow the instructions to sign it electronically if you agree. Save again to your desktop.
  • Next fill out the questionnaire. Save again to your desktop.
  • If you do NOT save to your computer after you filled out the forms, all your data could be lost. Always SAVE.
  • Email BOTH forms to AskDrYoung@gmail.com and then you can schedule your consultation.
